Use patient records to organize demographics, coverage, and care at a facility. This overview explains how to find patients and move from patient setup to clinical documentation.

Understand patient records​

Each patient record belongs to one facility. It holds the patient's demographics, contact information, patient details, tags, and notes.

Care and billing information connect to the patient in different ways:

  • Coverages describe payors, coverage periods, benefits, and authorizations for the patient.
  • Admissions record periods of care at the patient's facility. Each admission contains:
    • Billing episodes, which identify the primary payor, billing provider, dates, and therapy services to which they apply. They are separate from patient coverages.
    • Therapy cases, which organize one discipline's clinical documentation.

A patient's clinical documentation exists within an admission, organized into therapy cases for each discipline providing care. Open the relevant admission and therapy case to view or add clinical documentation.

Where to find patient records​

  1. Open People and select Patients to browse patients across the facilities you can access.
  2. Search by patient or facility name. Use Sort by name or Sort by facility, or click a tag to filter the list. Click the tag again to remove that filter.
  3. Turn off Active only if the patient does not appear. It is on by default and shows only patients with an admission that has no discharge date.
  4. Click the patient name to open Details, or click an admission or case badge under Admissions & Cases to open it directly.

For a single facility's list, open Facilities, select the facility, and click Patients. This list also enables Active only by default, so turn it off to find patients without an active admission.

After opening a patient, use these tabs:

Patient workflow​

A typical setup and care workflow is:

  1. Open Facilities, select the facility, and open Patients. Click New patient, enter the patient's details, and click Save. RehabAlpha opens New admission.
  2. Add coverages when coverage details are available. To do this before completing the admission, open Coverages, then return to Admissions and click New admission.
  3. Create the admission when the patient starts a period of care at the facility.
  4. Set up billing episodes within the admission when your organization tracks billing setup in RehabAlpha. If an episode will apply to specific therapy cases, create those cases first, then edit the admission to select them.
  5. Start therapy cases for each discipline providing care.
  6. Document evaluations, treatments, progress reports, re-evaluations, and discharges.

You should know​

  • Saving a patient creates the record even if you leave New admission without saving an admission.
  • Patients cannot sign in to RehabAlpha and cannot be invited as users.
  • The organization-wide directory is for finding and opening records. Create patients from the facility's Patients page; creating or editing patients requires write access to the facility's clinical data.
  • Patient deletion is under Settings → Danger zone. RehabAlpha blocks deletion while admissions or coverages exist. Follow Manage patient records for editing and deletion instructions.

Frequently asked questions​

Can patients sign in and see their information?​

No. Patients cannot currently be invited as users or sign in to RehabAlpha to view their information.

What if the same person receives care at two facilities?​

Create a separate patient record at each facility. Patient records cannot be moved between facilities.
