Write a discharge

Use a discharge to close a therapy case and summarize outcomes, remaining needs, and recommendations.

Before you start​

Review:

  • the evaluation
  • the most recent progress report or re-evaluation
  • goals and final measures
  • treatment history
  • discharge reason
  • recommendations for follow-up care

Complete the discharge​

  1. Open the patient's therapy case.
  2. Click Discharge.
  3. Enter the discharge date.
  4. Document goal status and patient progress.
  5. Add discharge recommendations.
  6. Save the discharge.

After saving​

Confirm that the case status, schedule, and open notices reflect that the therapy episode has ended.
